Financial Responsibility

A $50.00 non-refundable application fee is due at the time of application for enrollment. Your application will not be processed until this fee is paid in full. *Please be aware that full participation in esthetic treatments, both giving and receiving, is a requirement for satisfactory progress and course completion. Any condition that may prevent full participation must be disclosed fully prior to your enrollment. Please attach a physician’s certification describing the condition and how it would affect your ability to fully participate. Without a physician’s certification, failure to participate fully may adversely affect your grades and may even affect your ability to complete the course and graduate from Mandalyn Academy.
